The SurgiSculpt® Difference

GMommy BBL

A 61 year old female who had high definition liposuction and a Brazilian buttock lift

A GMommy BBL treats a deflated, sagging buttock with fat transfer and a lateral thigh and buttock tuck. Cellulite dimples need the tuck. At SurgiSculpt® in Newport Beach, our surgeons tailor each plan with artistry, proven protocols, and patient safety as the priority.

What a GMommy BBL Is Treating

A GMommy BBL is for buttock cheeks that are deflated and sagging at the same time. Deflation comes from hormonal change. Sagging comes from skin redundancy. Both follow prolonged gravity. A traditional fat transfer replaces volume. It does not lift a cheek that has already dropped, and it does not remove the extra skin. That is why this operation has two parts, not one.

The first part is fat transfer, the same transfer used in a traditional Brazilian buttock lift. Fat is taken by liposuction, including high definition liposuction of the back in the case above, and moved to the buttock. The graft stays in the subcutaneous fat. It is not placed in the glute muscle. The second part is a lateral thigh and buttock tuck. A wedge of skin and soft tissue is removed from the upper buttock. That wedge lift tightens the cheek skin. Volume and lift are planned together because the cheek is short on both.

Why a GMommy BBL Includes a Tuck

The usual sign of extra skin and soft tissue on these cheeks is cellulite. The dimples are not a separate disease from the sag. Redundant skin droops, and retaining cutaneous ligaments pull the skin inward at the center of each dimple. The result is the checkerboard look. Filling a dimple with fat, or treating it with a minimally invasive device, is often proposed. The only definitive correction of that checkerboard is the lateral thigh and buttock tuck, because the tuck removes the redundant skin the ligaments are tenting.

Fat grafting still has a job. It restores the deflated cheek. It does not replace the wedge. A GMommy BBL that transfers fat and skips the tuck leaves the sag and the dimples in place, with more volume behind them. A tuck that skips the transfer lifts the skin and leaves the cheek empty. The 61 year old back view shows high definition liposuction and a Brazilian buttock lift. The caption does not itemize the wedge. The exam is what decides whether the tuck is required.

The Lateral Thigh and the Hip Dip

A GMommy BBL also treats the lateral buttock. That area is sagging plus fullness of the lateral thighs, which produces a prominent lateral hip dip. The dip hurts the buttock outline and the hourglass line of the waist and hip. The lateral thigh tuck, which is part of the buttock tuck, lifts the redundant skin and the fullness over the lateral thighs and fills the lateral hip dips at the same time. The dip is not a hole that only a graft can patch. It is skin and fullness that the tuck repositions.

Projection from the fat graft is central and subcutaneous. It adds the volume hormonal change took away. It does not set the upper-cheek lift, and it does not set a lower pole by itself. The wedge is what changes the dropped upper buttock. If the missing shape is only a pole that needs fat, and the skin is not redundant, this operation's tuck is not the finding. The GMommy pattern on this page is both findings together: deflation and sag.

How a GMommy BBL Is Planned

The exam lists five changes, and a GMommy BBL is the plan that is built to meet the ones that are present: a deflated buttock, a sagging cheek, diffuse cellulite, lateral hip dips, and sagging, full lateral thighs. Fat transfer answers the deflation. The lateral thigh and buttock tuck answers the sag, the cellulite, the dip, and the full lateral thigh. Leaving one of those off the plan leaves that finding in the result.

Liposuction of the back and flanks is how the fat for transfer is collected, and it is also how fullness next to the tuck is reduced. The graft then follows the Safe BBL limit already used here: subcutaneous only, above the gluteus muscle. A GMommy BBL does not change that limit because the patient is older or because a tuck is added. The tuck is a skin excision. The graft is still fat in the subcutaneous layer.

The two parts also split the recovery and the fee. Fat transfer limits sitting while the graft settles. The tuck adds an incision across the upper buttock and the lateral thigh, so lifting and direct pressure on that closure are limited while it heals. Buttock augmentation cost for the transfer is not the same line as the tuck. The photographs are a back view after high definition liposuction and the buttock lift. They are read with the patient standing, at the BBL locations, so sag and deflation can be told apart before either part is promised.

Deflation and Sag Are Separate Findings

Deflation is lost volume. The cheek looks empty because hormonal change and years of gravity have reduced the fat that used to fill it. A graft can put volume back in the subcutaneous layer. Sag is extra skin that has dropped. The cheek hangs, and the upper buttock shows a fold a graft cannot cut away. Calling both findings by one name, and treating both with fat alone, is how the dimples and the drop remain after the size looks larger.

The wedge is taken from the upper buttock and continues into the lateral thigh. Removing that skin lifts the cheek and lifts the sagging lateral thigh in one excision. Fullness over the lateral thigh comes with the skin that is removed, which is why the same tuck fills the lateral hip dip. The dip improves because the drooping fullness is lifted, not because a separate pile of fat is packed into the dip. If the exam finds an empty dip without sagging skin, that is a different plan from the one described here.

The back-view photographs are one 61 year old woman after high definition liposuction and a Brazilian buttock lift. They show the back contour. They do not label each dimple, and they do not measure how much skin was excised. A reader should not treat the pictures as proof that every sagging cheek gets the same wedge. The ligaments, the redundancy, and the lateral fullness are judged in person, standing, before the wedge is designed.

Sitting limits belong to the graft. The new fat is subcutaneous, and pressure on it is limited while it settles. The incision limits belong to the tuck. The closure crosses the upper buttock and the lateral thigh, so strain on that line is limited while it heals. Those are two restrictions from two maneuvers. A garment can support the suctioned back. It does not hold a dropped cheek up if the wedge was never cut.

The Wedge and the Graft Stay Separate

The upper-buttock wedge is an excision, so the scar follows the line where the skin was removed. That line runs across the upper buttock and out onto the lateral thigh. Its job is mechanical. It takes out the extra skin and the soft tissue that hangs with it, then closes the remaining skin under less droop. A graft cannot do that job. Fat added under redundant skin makes the cheek larger and leaves the fold.

The fat that is transferred is prepared from liposuction of the back and flanks. It is placed only in the subcutaneous layer, the same limit used for buttock augmentation with fat. The wedge does not become a pocket for the graft, and the graft is not used to hold the closure. If the exam shows deflation without redundant skin, fat transfer alone is the matching operation. If the exam shows redundant skin without deflation, the tuck is the matching operation. Both findings together are what call for both maneuvers.

The standing exam is also where the hourglass line is judged. Waist and hip are read together because a full lateral thigh and a dropped cheek change the hip before the waist is even marked. Liposuction next to the tuck reduces the fullness that would otherwise sit above the new cheek. Projection from the graft is then added where the cheek is empty, not where the skin is simply loose.
