Patients who present desiring significant enlargement of their central buttocks following moderate to severe deflation of their bottoms and/or who do not possess adequate fat reserves are ideal candidates for buttock implants. Buttock, of butt implants are made of semi-solid silicone and possess a smooth or textured surface. This semi-solid design makes the implants resilient to daily mechanical stresses exposed by sitting on our bottoms. The textured surface ensures adherence of the implant to the soft tissues and stabilizes implant position by prohibiting migration and displacement.

Choosing the right Butt implants

Butt implants are inserted using well-hidden and limited-length inter-gluteal vertical incisions placed in a gentle arc to follow the natural curve of the buttocks cheek. Our Butt implant surgeons carefully dissect identical and symmetric intramuscular pockets where the silicone implants are to be positioned and inserted using minimal touch technique. Placement of the implant in an intramuscular (gluteus maximus) pocket provides several advantages, including an implant that is well cushioned so that its edges are not visible and not easily palpable, as well as minimizing the risk of infection or implant rejection. Minimizing the risk of infection is ensured by meticulous attention to sterile technique, non-traumatic surgical dissection, and multi-layered closure of the incision lines following the placement of butt implants. Avoiding extrusion is best accomplished by choosing an implant that is not too large for your buttocks cheek. This is often the biggest obstacle for patients desiring buttocks augmentation. Most clients want to go as large as possible. However, the buttock implant volume, specifically the diameter, must fit in your gluteus maximus muscle. The gluteus maximus muscle pocket can be best defined by measuring the distance from the vertical gluteal crease to the start of the lateral gluteal depression. This distance subtracted by 2 to 3 cm is the maximum implant diameter that your buttock can accommodate. Then this diameter is used to choose your buttock implant from the limited number of buttock implant sizes available. You see buttock implant surgery has not been universally accepted by board-certified plastic surgeons. There are several reasons for this hesitancy. First, there is a high extrusion rate nationally which results from implant size chosen that is inappropriately too large for your buttock and poor compliance postoperatively. This is because the recovery time is prolonged since patients must give adequate time, in the range of 6 to 8 weeks, for the incision line to heal before initiating physical activity. Buttock Implant Candidates

Butt implant surgery is an alternative to the Brazilian Butt Lift, a buttock augmentation surgery that involves fat injections. Fat transfer surgical procedure has the advantage of being able to augment the butt cheeks globally but does have the disadvantage of limited central buttock filling. This is why buttock implant augmentation positioned in the gluteal muscles remains a viable cosmetic procedure for patients desiring maximum central buttock projection. A butt implant procedure may be considered if you are a strong candidate for butt implants. Candidates for butt implant surgery will often have a very low body mass index with a history of extreme weight loss and present with excess skin and minimal fat deposits. Plastic surgeons must counsel patients regarding the constraints of possessing butt implants, considered a less desirable alternative to Brazilian Buttock Lifts. Long-term butt implants can be prone to malposition years following implantation.

Butt implant Surgery Postop Considerations

Butt implant surgery patients will wake up from anesthesia to find themselves wrapped in a compression garment. Under the garment, they will have a customized foam in-lay positioned to protect the buttock from unnecessary compression. Following the placement of silicone butt implants, patients will feel moderate pressure over their buttock cheeks. They will also feel tight along their buttock crease where surgical incisions are positioned. Do not worry about popping your incision, as your incisions are well supported with multiple layers of stitching and sealed with a surgical tape called a Steri-strip. However, your post-surgery instructions are very strict with minimizing physical activity. Butt implant patients must remember that they have artificial implants made out of silicone. Ultimately, your body considers this a foreign element regardless of the type of implant. An alternative to butt implants is fat grafting injections that are completely autologous. Regardless, the benefits of buttock implants are unique in that they provide maximum central buttock projection, especially in patients with a void of fat volumes. Patients undergoing buttock augmentation with implants must commit to a prolonged recovery that involves the creation of scar tissue, called a capsule, that will form around what the body considers a foreign body. Once the capsule forms, the implant is protected from extrusion and malposition in the early postoperative period. Delayed complications of butt implants remain implant malposition. This complication results from the movement of implants from continual forces sitting on your implant. Years and even decades following your buttock implant procedure, you may observe deformity of your buttock shape as the implant migrates out of its intramuscular position. Augmentation with butt implants requires a commitment. This means being cautious every time you sit and avoiding exercises that strain the gluteus muscles, such as squatting. The buttock implant procedure is not for everyone, requiring strict compliance with postoperative protocols. Unfortunately, buttock implant malposition resulting from pocket dilatation can only be treated with butt implant removal. Patients who undergo butt implant removal may consider both partial refilling using fat as well as a gluteal lift intended to reduce buttock skin redundancy and sagging. Patients who favor buttock implant augmentation over a Brazilian butt lift procedure are urged to consider their daily and physical activity levels.

Finally, you will need to arrange for a friend or a spouse to care for any small children who may need to be lifted since you will be sore for the first couple of days following surgery.

Patients are allowed full physical activity and wearing of routine undergarment four weeks following surgery.

Using two separate incisions rather than a single one has decreased the incidence of implant infection and extrusion.
