Ultimate buttock augmentation requires lateral hip filling and central and mid-buttock fullness. Mid-buttock filling has become a routine part of all Brazilian buttock lifts. However, with advances in buttock aesthetics, it has become evident that the lateral hip and thigh contour is intimately associated with the aesthetics of the buttocks.

Specifically, ideal female buttock contour lines require smooth transition contours between the buttock and legs via a properly contoured lateral hip region. As such, a lateral hip filling has become integral to achieving ideal buttock aesthetics.

Lateral hip filling surgical approach

To optimize lateral hip filling, several chronologically appropriate steps are taken. First, the mid butt is filled to define the peak of the lateral contour line that defines the hourglass appearance. Next, the lateral hip region is filled using fat transfer protocols.

Finally, the upper lateral thighs are conservatively defatted to provide a smooth transition from the buttock proper to the lateral hips and upper thighs and down to the mid-lateral thighs. Once again, all efforts are made to create smooth transition lines that are critical to achieving optimal female contour aesthetics.

Ideal lateral hip fat transfer

Ideal lateral hip fat transfer is unlike buttock filling since the lateral hip extends to the back and the front. Whereas the buttock can is optimally filled while the patient is placed on her stomach, the lateral hip region must be approached from the back and front. Routine filling volumes include approximately 100 to 150cc of injection from the back and another 50 to 100cc of fat transfer from the front.

Actual volumes of fat transfer are dictated by the degree of mid-butt filling and how generous the upper lateral thighs are. As a result, optimal lateral hip fat transfer is achieved with the patient positioned both on their stomach and back. This bimodal filling technique and patient positioning allow us to optimize the lateral hip contour to be smooth in appearance.

Lateral hip filling summary

In summary, lateral hip filling requires attention to both the front and back side of the hip as it is located over the lateral aspect of the body. Only by filling from both the front and back side can optimal lateral hip contour be appreciated. In addition, ideal buttock augmentation requires lateral hip filling to create a smooth transition from the peak mid-butt down to the legs.
